TOPIC: Cardiothoracic Surgery TYPE: Fellow Case Reports INTRODUCTION: Pulmonary artery catheters (PAC) are used in cardiac surgery to provide critical clinical data. While safe, they not harmless with multiple associated risks, including surgical entrapment. Causes of include suture snaring and PAC knotting. We present the case a surgically entrapped that was successfully removed percutaneous fashion. CASE PRESENTATION: A 61-year-old man MV regurgitation presented for repair. PA catheter placed through an 8-FR sheath RIJ vein. Sternotomy performed patient on central cardiopulmonary bypass bicaval venous cannulation. The LA entered inter-atrial groove, repair performed. Following both atriotomy closure decannulation, anesthesia team reported seamless mobility chest closed. On POD1, removal met resistance. Imaging confirmed normal position without obvious taken IR suite where fluoroscopy revealed tethering at inferior third SVC. proximal aspect then cut indwelling replaced 9-FR braided over PAC, which served as 'guide-wire'. point obstruction detected by tactile sensation. upsized 10-FR vascular sheath. Entrapment of a Swan-Ganz catheter.
Failure to withdraw a catheter is related primarily to catheter entrapment or knotting. Placement of a Swan-Ganz catheter is associated with various complications, one of which is entrapment. Entrapment of a Swan-Ganz catheter in the heart, vena cava, or pulmonary artery is a very rare and serious complication that may lead to life-threatening complications if not treated appropriately. Most re...
متن کاملMultiple knotting of a swan-ganz catheter.
Complications derived from pulmonary artery catheter placement are about 3%-17%,1 from the most frequent and mild (carotid puncture, extrasystoles, etc) to the most severe (pulmonary artery rupture, pulmonary infarction, ventricular fibrillation, atrioventricular block, etc), and rare ones, such as intravascular catheter knotting, located preferentially in the right atrium (RA) or right ventric...
متن کاملInadvertent fracture during the attempted removal of entrapped Swan-Ganz catheter.
Dear Editor, Pulmonary artery catheter is widely used in cardiovascular surgical practice. Various complications of the Swan-Ganz catheter during insertion, its long-term placement, or removal are well known since its introduction.1-5 Herein, we report a case of the inadvertent fracture of an entrapped Swan-Ganz catheter during its attempted removal with moderate external traction. A 36-year-ol...
متن کاملA Noninvasive Alternative to Swan-Ganz Catheter
Background—Although several studies have demonstrated a good correlation between Doppler echocardiographic and invasive measurements of single hemodynamic variables, the accuracy of echocardiography in providing a comprehensive assessment in individual patients has not been validated. The aim of this study was to assess the accuracy and clinical applicability of Doppler echocardiography in dete...
